New Patient Registration
We book you an hour and half for your first appointment. First appointment is generally bulk-billed. If you are booking for a dependent child, please complete two registration forms, one for your child and one for yourself. To be able to treat a child well, it is essential to get to know the parent/s or carer and work together to help the child.
Please fill in your new patient registration form and bring it with you to your first appointment. Please also bring your driver’s license (or other photo ID). Please ask us if you need any clarification or help in filling it in.
Please remember to bring all your prescribed and non-prescribed medications including herbal, over-the-counter medication, creams, inhaled and injectable medications (like a puffer or insulin pen), supplements, eye or ear drops and any other form that you may be taking.
Please remember to bring with you all your available test results including blood test, x-rays and other imaging. Also bring with you all your specialist reports and any other reports or investigation results that may help.
